We do not offer Pain Management.
We will not fill pain/narcotic prescriptions.
New patients, please arrive 30 minutes prior to your appointment time. If you are a new Medicare patient, please arrive 1 hour prior to your appointment time.
This will allow us to prepare your chart get all the information we need to bill your insurance.
We must have a copy of your insurance card and state issued ID at the time of your visit. This will allow us to ensure your insurance is billed properly. If you do not have this at your appointment time, you will be rescheduled.
We collect your copay, deductible, and co-ins due at the time of service.
It is your responsibility to know the benefits and limitations of your insurance policy. If any prior authorizations are required or you must use a preferred lab, please let us know before the procedure is performed or you may be billed the entire cost of the procedure.
Private Pay patients are required to pay $108 at the front desk before your visit. We will settle the bill for any other services performed at checkout.
